• The agreement starts as soon as the member has agreed to the terms and conditions and the. first monthly fee has been made.
• Membership starts on the agreement date.
• Membership is for a period of 6 months from the membership start date, as agreed by the member.
• During the term of this agreement cancellation may only be considered as per the Fair Cancellation Policy below.
• Members are entitled to all rights and privileges of the membership.
• This Agreement is non-transferable.
Fees and Charges
The fee relates exclusively to the monthly membership fee, which includes
doctor consultations and all associated treatment medications and supplies as outlined in the patient treatment proposal. Additional supplements, medications, and labs associated with the membership are deemed an addition to the fee and are subject to change.
1. All payment for membership is due in advance as follows:
a. Monthly recurring memberships: An initial payment for the balance of the first month’s membership, followed by advance payments which will recur monthly.
2. Initial payment is due and payable immediately before any prescriptions are sent to the compounding pharmacy. These payments are non-refundable except in circumstances where: a. There has been a breach or negligence by us or b. On the valid exercise of your statutory cancellation rights.
3. All monthly membership payments shall be paid by credit card and are payable monthly in
advance.
4. The number of monthly payments, as specified by your membership, are required before this contract may be terminated. Once you have completed the required number of monthly payments you can cancel your automatic renewal payments anytime by notifying Revv health in writing, which can be done via email.
5. If you fail to pay any amount due under this agreement, all prescriptions will be withheld until the amount due has been paid in full.
6. You agree to advise us promptly of any change to the member details provided.
7. A member whose membership is terminated shall forfeit all the privileges of membership with immediate effect, there will be no entitlement to a refund of any monthly membership charges already paid.
8. Monthly membership fees may be increased, subject to 30 days notice, at the discretion of Revv Health.
Membership Renewal
9. Once the terms of initial membership period are satisfied, the membership continues on a
rolling monthly recurring payment until canceled by either party.
10. At the end of the initial membership period you may prevent automatic renewal at any time
by giving 30 days' notice in advance.
Fair Cancellation
11. Revv Health operates a fair cancellation policy. Only under the following circumstances will we consider the cancellation of a membership before the term of the membership has ended.
12. Long-term (over 3 months) illness or injury: This agreement may be canceled in the event of
an illness, injury, or medical condition which would prevent the patient from continuing with their existing treatment protocol. Appropriate proof must be provided.
13. Hardship: This agreement may be canceled where the members financial circumstances
have reduced to the extent where it makes it difficult to sustain the membership. Appropriate
proof must be provided. Please note – cancellation for the above reasons will not be affected until the appropriate proof is received in writing or via email.
14. Breach: This agreement may be canceled if Revv Health is in breach of contract. This
includes not providing services which may reasonably be expected by the member, or
where standards fall below that detailed in the membership terms and conditions.
