I understand that hormone therapy will be considered based on my symptoms, health history, laboratory findings, treatment goals, and my provider's clinical judgment.
Depending on my individual needs, treatment may include estradiol, progesterone, testosterone, thyroid hormone, DHEA, pregnenolone, or other therapies considered appropriate by my provider.
I understand that individual response to treatment varies and that no specific outcome can be guaranteed.
The potential benefits and risks of hormone therapy vary depending on the hormone, dose, route of administration, my individual health history, and other factors.
Potential effects or risks may include breast tenderness, vaginal bleeding or changes in menstrual bleeding, acne or oily skin, hair growth or hair loss, fluid retention, changes in libido or sexual function, mood or sleep changes, changes in heart rate, and changes in cholesterol or other laboratory markers.
I understand that estrogen therapy may have risks and benefits that vary depending on my age, health history, treatment timing, dose, and route of administration. If I have a uterus and estrogen is prescribed, I understand that progesterone may also be recommended to protect the uterine lining.
I understand that testosterone therapy in women may cause androgenic effects including acne, increased facial or body hair, scalp hair loss in genetically susceptible individuals, or other androgen-related changes. Some androgenic changes may not completely reverse after stopping treatment.
If thyroid hormone is prescribed, I understand that excessive thyroid hormone may cause symptoms including rapid heart rate, palpitations, tremor, anxiety, sweating, sleep disturbance, or other symptoms of excessive thyroid hormone exposure.
I understand that hormone therapy should not be used during pregnancy unless specifically determined appropriate by a qualified healthcare provider. I agree to notify REVV Health if I become pregnant or believe I may be pregnant while receiving treatment.
I understand that appropriate laboratory monitoring and follow-up are important components of hormone therapy and that my treatment may be adjusted based on my symptoms, laboratory findings, response, and safety considerations.
I understand that some hormone optimization treatments may involve medications, formulations, doses, or therapeutic goals that differ from conventional approaches or may involve off-label prescribing when considered medically appropriate by my provider.
I have had the opportunity to ask questions regarding treatment, potential benefits, risks, alternatives, and the option not to pursue treatment. I voluntarily consent to hormone therapy when prescribed by my REVV Health provider.
