Effective date: October 14, 2026
REVV Health (“REVV,” “we,” “us,” or “our”) respects the privacy of visitors, prospective patients, and patients. This Privacy Policy describes how information is collected, used, stored, and shared through our website, online intake forms, and related website communications. It does not replace any separate notice or authorization that may apply to clinical records or particular services.
When you contact us or complete an intake form, we may collect identifying and contact information such as your name, date of birth, address, telephone number, and email address; health information such as symptoms, medical history, medications, treatment history, reproductive history, and treatment goals; and information you include in messages to us.
Our website may also collect technical information such as IP address, device and browser details, pages visited, and information generated by cookies or similar technologies. The precise tracking tools and settings are subject to confirmation.
We use information to respond to inquiries, review prospective-patient intakes, arrange consultations, support care and related administration, maintain records, operate and secure our website, and meet applicable legal and professional obligations. Submission of an intake form does not by itself establish a provider-patient relationship.
Our prospective-patient intake forms are built using WordPress/Elementor. Completed submissions are stored temporarily in the website database. Access to submissions is limited to authorized REVV personnel through password-protected accounts.
When a form is submitted, an email notification containing information provided in the completed submission is sent to designated REVV Health personnel.
After REVV downloads a completed submission into the patient’s secured chart, the submission is deleted from the active WordPress database. Routine website backups may retain copies for a limited period until those backups expire or are replaced. Backup retention periods have not yet been confirmed.
We may provide information to authorized members of our care team and to vendors that support website hosting, forms, communications, patient records, or other operations, as necessary for their services and subject to applicable law. We may also disclose information when required by law or to protect legal rights and safety as permitted by law.
We do not use this policy to authorize the sale of patient medical information. Any data-sharing and advertising practices must be reviewed against actual website configurations and applicable law.
The website currently contains Google Tag (gtag) code. Depending on configuration, this technology may support analytics or other Google services. We have not yet verified which services are enabled or what data they receive.
Cookies and similar technologies may be used for essential site functions, performance measurement, and other purposes identified after a technical review. Where required by applicable law, REVV will provide appropriate notice and choices. Sensitive intake-form information should not be transmitted to advertising or analytics services.
Intake submissions are removed from the active website database after transfer into the secured patient chart. Copies may remain temporarily in routine backups, depending on backup settings and retention cycles.
Clinical records are maintained separately in accordance with applicable recordkeeping requirements. Other website or administrative information may be retained as reasonably necessary for operations, security, and legal obligations.
REVV limits access to WordPress intake submissions to authorized personnel using password-protected accounts. We seek to apply safeguards appropriate to the sensitivity of the information we handle. No electronic system or transmission can be guaranteed completely secure.
Please do not submit urgent medical concerns through website forms or ordinary email.
Our website uses WordPress and may rely on third-party providers for hosting, form functionality, email notifications, patient records, and other services. Our website may link to external services, such as a patient portal and podcast platforms. Their privacy practices are governed by their own policies.
You may contact REVV to ask questions about information you have provided, request corrections, or raise a privacy concern. We will respond as required by applicable law and may retain information where medical-record or other legal obligations apply.
The general website is not intended to solicit personal information from children under 13. If you believe information was submitted by a child without appropriate authorization, please contact us.
We may update this policy as our website, services, or legal obligations change. The updated version will be posted with a revised effective date.
